UnityPoint Health and CareSignal Partner to Optimize Chronic Care Management Through Deviceless Remote Patient Monitoring
Each year, 20 percent of a healthcare system’s rising-risk patients experience a serious health decline and become high-cost, high-risk patients. Health systems make constant efforts to improve patient outcomes and control healthcare costs, but existing care management processes can be time consuming and ineffective due to time spent cold-calling patients and poor patient engagement.
As value-based care contracts continue to expand, the need for a scalable and sustainable solution to manage care for high-risk populations has become even more critical. UnityPoint Health recognized an unmet need to provide proactive patient outreach to high and rising-risk patients and chose to invest in CareSignal due to several unique factors.
- CareSignal offers Deviceless Remote Patient Monitoring (Deviceless RPM), providing clinically actionable patient engagement for populations with chronic and behavioral health conditions, identifying actionable moments for care management teams to deliver proactive patient care.
- Evidence-based programs for more than 30 conditions, from CHF and COPD to breastfeeding adherence and Social Determinants of Health, leverage the technology patients already own and use, capturing patient-reported outcomes via SMS (text message) and phone call. The clinical and financial impact of these programs is evidenced by over 10 peer-reviewed journal articles and 19% reduction in global claims for populations using CareSignal.

CareSignal expands upon existing care management programs and improves patient care without increasing workload or costs for over two dozen common conditions, including diabetes, depression and asthma. CareSignal’s Deviceless RPM prompts patients to report their health status via text messages or phone calls. From there, care teams receive alerts enabling real-time prioritization and outreach. CareSignal amplified the utility and value they bring as a partner by extending beyond initial scope and providing programs that address challenges associated with the COVID-19 pandemic. Early in the crisis, CareSignal released platforms allowing health systems to remotely monitor employees who were exposed to or tested positive for COVID-19 and monitor low acuity COVID-19 patients whose condition could be safely managed from home.
